New Patient? How to Get Ready for Your First Visit

A little preparation makes your first appointment far more useful.

The easy way: when you book, ask our staff to send you the new-patient registration email — it contains a fillable form covering your contact details, medications, health history, and family history, so everything is ready before you arrive.

On the day, bring with you: your Alberta Health Care card (visits are covered by AHCIP), photo ID, and — if you have them — your medication bottles and any recent results or records from a previous doctor (we can also request transfers for you).
